Unused H1b approval notice

Hi,

Iam a Master Degree Holder with 5 years of IT experience. I had applied for my H1B petition in 2013 April through a consulting company in US. Meanwhile I travelled to US in H4 in June 2013. My H1B petition got approved in Oct 2013 for 3 years [2016, Sep]. When the consulting company applied for change of status from H4-H1b showing an in-house project it got rejected due to “employer-employee relationship not established”. My consulting company is considered as a “job shop” as it does not have a valid inhouse project to prove that they can hire/fire and mainly assign tasks to me as an employer.

1.What options do I have now?
2. Can I apply for H1B transfer with any other company who can establish the required “employer-employee relationship”?
3. Can I re-apply under cap-exempt with any other company who can establish the required “employer-employee relationship”?

I only have a copy of my H1B approval notice.
