Name different in Academics and Passport

My name is different in academics and passport.
Name in academic certificates:

First Name N D Srinivas
Last Name: U

Name in Passport:

First Name: Naga Durga Srinivas
Last Name: Udarel

Is this ok or do i need an affidavit for filing H1B petition/registration?

You need to ask this to your H1B employer’s immigration lawyer.
Generally speaking your foriegn degree evaluator will look for same name on your ID and the degree.
