My spouse is on H4 visa in USA. She doesn’t have EAD. If I apply for her H1 and if it gets picked up in lottery and if the petition gets approved, can she start working on H1B from 01 Oct 2023 based on the approved petition or does she need to undergo ‘Change of Status’ first, and can work only after it gets approved?

H1B can be applied with change of status. Make sure the employer knows that your spouse will need change of status.
