My transcript mentions no. of attempts taken to complete the sem as 2 instead of mentioning which subject. How will the backlog be evaluated with only that detail? Do they ask for additional document like mark sheet other than transcript? How will they count? Should I mention about these in my SOP?

Well, they may or may not ask. It is more asked verbally during visa interview.
There is no need of proof or anything that you had to take it twice.
Do not worry about it too much.
You do not have to mention it in SOP, but you can mention it stating your struggles and your passion to study…
