H4EAD Form section not applicable should be Blank

Hi I’ filling H4EAD form I-765 for my Spouse first time. The form has lots of sections which are not applicable to us such as 28, 28.a, 28.b, 30.d, 30.e, Half of the Page 4 and Page 5-7.

Should I leave them Blank and mark N/A ? Please guide.

Also, does any kind Cover Letter required along with H4EAD application.

Not required but good to have one with the list of documents you will attach with the I765 form including the payment.
