H4-H1 COS approved but got i797A on older i94

i’m On H4 and in 2017 April I have applied for H1b nd got picked up in lottery.while it was in process I needed to travel outside the country nd came back in August.We have received RFE in October ,while replying to response we have submitted new i94. Recently 1 week back received h1 approval notice i797A with change of status approved but with the older i94(which we have submitted for filing h1) .

My question is

  1. Will this be a problem to apply SSN.

  2. Do I again need to apply for COS
