H1b approved and effctive from oct 1st but needs maternity leaves

HI , I need an urgent advice. I have an approved H1b which is effective from oct 1st 2013. But I am pregnant and and delivery is due is december. There are some medical complications that I am facing. Is there a possibility that I go on maternity leaves fom oct 1st and join in Februaury . Is it necessary to join first and get a paystub to go on maternity leaves . Please advice

If you are in US if the petition is approved with COS you need to join.

If petition is approved without COS you need NOT join.
