Below is case for my spouse(Didn’t mention real names)
Didn’t hold any passport with maiden name.
Maiden Name : Sunita Damodar Joshi (as in Marriage Certificate)
Name after marriage : Anita Srikant Phadke (as in Passport) legal name after marriage with Government gazette)
Husband Name being : Srikant Phadke
For H1 DS 160 , in Travel Companion section , it is mentioned to add details as per passport. But in Family Details section it is mentioned to include Maiden Name.DS 160 doesn’t allow to add any special characters(-,/) etc.
What name should be populated in the Family Detail Section.
