Typically applying to PhD involves taking standardized tests as required by the schools like GRE/ TOEFL or subject GRE. You need to write Statement of purpose or essay for the school and complete application process as required by the school. Each of these vary slightly by school. You need to check few of the universities websites to get some idea on the requirements. Before all of this, are you sure you want to do PhD ? What is your goal ? Check Goal for MS in US PhD .
