Filling questionnaire for h1b dropbox waiver

1. Have you ever been refused a U.S. visa before?
   My current visa is H1B and I have applied for L1B visa before and was refused. Shall I answer yes or NO to this question.

2. Did your most recent application result in you being issued the visa?
   If I answered “Yes” to previous question, then what should I answer for this? My H1b was issues in 2015 and my L1b visa was refused in 2011.

If i answer to “N” to 2 question then i am not getting waiver where answering “Y” is giving dropbox waiver.

Please suggest.

Most recent H1B visa was approved so you should answer yes.
