Approved H1B petition & travelling to US in H4.

Hi,

My Spouse is currently in India without any visa while I'm in US on H1b. I'm planning to file H1b for my spouse through a consultancy company this april & I'm thinking of bringing my spouse in H4 in November 2014

 

My questions are

 

1. "If everything goes well and my spouse gets an approved H1B without COS, Can she travel in H4 with an approved H1B Petition?

 

2. Once she is here,COS have to be filed or would it be new H1B(Which doesn't fall under the cap count)?"
