Hello, currently I am on L1A visa and I am in US since last 4 years. If my H1B gets approved by october then do I have to Visit India to get H1B visa stamped or can I continued to stay here withoug vising India.

I think you could do a Change of Status from L1 to H1 and you do not have to go for stamping again.
You should check with your employer if they have filed the H-1 petition w/ COS or not. If approved w/ COS, you will move to H-1 status automatically from Oct 1 or H-1 approval date (whichever is later) and can no longer work on L-1A thereafter. If COS is not filed, then a separate COS needs to be filed or you need to enter on stamped H-1 visa.
