I recently got h1b approved, however it is attached to old i94. Even after submitting new. I don’t know whether my employer/attorney did that mistake or from h1b process. What do I need to do?

When you say attached to old I-94, do you mean the attached I-94 has the old number (i.e. your current I-94# which was issued when you entered US)?
When a new H-1 is approved w/ COS or EOS, the attached I-94 will have same number as previously issued I-94. However, it will have extended expiration date to match 797 expiration date and will mention the classification as H-1B. Is that not in your case?
