H4 EAd Application rejected

Hi

My H4 EAD application was rejected due to a wrong date in the check. USCIS has requested to resubmit with a new check.

Do we need to submit with a new form or the old filed up form will do. The old form is two weeks back dated

Same package with correct check.

Two weeks should be fine. If you really worried, re-fill it and submit. But, it does not matter.
