I got H1 RFE on my CPT evidences and F1 status Maintainance evidences, I could able to answer all the questions except one final question: A statement explaining how the beneficiary obtained his CPT employment (individually, through the school or through an outside company or consultancy)

I would be an example of such a letter.

You check with your school on the CPT eligibility and submit the documentation from school and then details of the CPT related employer, including letter. If you are in Day 1 CPT or anything as such, you need to be careful. I suggest you work with your H1B filing attorney and submit them carefully and if they believe that it cannot be sufficient, convert it to consular processing.
