My H1B is approved however, got the approval only for 1 year, does that mean my total H1 B duration can only be 4 year(1 year now and assuming i get the extension for 3 more year)

One can live/work on H1B status for a maximum of 6 years. This doesnt mean that you get the max allowed time for your visa approval. Your visa allocated time depends on your employment sponsorship details rather than howmuch time you are left to be on that status. You need to apply for extensions each time your status expires to a max of 6 years.
